The short answer

MEDVi is $179 for the first month and then $299 a month for compounded semaglutide, while Direct Meds charges a flat $297 a month from the start, so the ongoing cost is effectively the same and MEDVi is about $118 cheaper in month one. Both have received FDA warning letters about how they promoted compounded GLP-1s, MEDVi in February 2026 and Direct Meds in September 2025.

After the first month the two cost almost the same, so the decision turns on terms and track record. MEDVi wants 72 hours’ notice to cancel. Direct Meds wants 48 hours before the next processing date, and does not serve Mississippi or Louisiana.

MEDVi’s first month is $179, which makes it about $96 cheaper across the first twelve months. Tirzepatide is $399 a month at Direct Meds.

Did the FDA send warning letters to MEDVi and Direct Meds?

Yes, to both. MEDVi, LLC received one dated February 20, 2026 and Direct Meds one dated September 9, 2025. Both concern false or misleading promotion of compounded semaglutide and tirzepatide, such as implying equivalence to FDA-approved drugs. We have not established the current status of either.
