Foundayo vs Wegovy Pill: Which GLP-1 Pill Fits You?

Last reviewed: 1 October 2026

A blister pack of white tablets on a plate
Short answer

Both are once-daily GLP-1 tablets approved for weight loss in adults with obesity, or overweight with at least one weight-related condition. In their label trials, Wegovy tablets (semaglutide 25 mg) averaged 13.6% weight loss at 64 weeks and Foundayo (orforglipron) up to 11.1% at 72 weeks; the trials weren’t head-to-head. Wegovy tablets must be taken on an empty stomach with a 30-minute wait; Foundayo can be taken with or without food. Both start at $149 a month self-pay. A licensed clinician decides which, if any, suits you.

Which works better?

No one knows, because the trials were separate. In its label trial, Wegovy 25 mg tablets produced an average 13.6% weight reduction at 64 weeks against 2.4% on placebo, and 76.3% of treated patients lost at least 5% of their body weight. Foundayo produced 7.4%, 8.3% and 11.1% at 5.5, 9 and 17.2 mg over 72 weeks, against 2.1% on placebo.

Three cautions apply before you read that as a ranking. The two trials enrolled different patients and ran for different lengths. Both paired the drug with a reduced-calorie diet and increased physical activity. And an average hides a spread; the labels report that some treated patients lost less than 5%. These are not typical individual results, and the figures come from each drug’s own placebo-controlled trial, not from a comparison between the two.

The semaglutide tablet is the same active ingredient as the Wegovy injection, which averaged 14.9% at 68 weeks in its own trial; GLP-1 pill vs injection puts the tablets beside the injections.

How do you take each one?

Both are one tablet a day, and the rules around that tablet are where they differ most. Wegovy tablets must be taken “on an empty stomach in the morning with water (up to 4 ounces)”, swallowed whole, followed by a wait of “at least 30 minutes before eating food, drinking beverages or taking other oral medications”. The label explains why: the tablet is co-formulated with an absorption enhancer called SNAC, absorption happens mainly in the stomach, and only about 1% to 2% of the semaglutide is absorbed.

Foundayo can be taken “with or without food”. Its label says to swallow tablets whole and not to break, crush or chew them. Neither label allows more than one tablet a day.

If your mornings are irregular, or you take other morning medications that cannot wait 30 minutes, that difference will matter more to you than the trial averages.

How fast do you reach the full dose?

Both escalate gradually “to reduce the risk of gastrointestinal adverse reactions”, and both take about three months to reach a maintenance dose.

Self-pay prices rise with these steps, so the dose you start on is the cheapest you will be on. Why the price goes up after the first months covers every manufacturer’s dose tiers.

What do they cost?

Both start at $149 a month at the manufacturers’ self-pay prices, read October 1, 2026. Novo Nordisk prices the Wegovy pill at $149 a month for 1.5 mg, $199 for 4 mg and $299 for 9 mg and 25 mg; one month is one bottle of 30 tablets.

LillyDirect prices Foundayo at $149 a month for 0.8 mg and 2.5 mg, $199 for 5.5 mg and $299 for 9 mg. The 14.5 mg and 17.2 mg doses are $299 “when you refill within 45 days” under Lilly’s Foundayo Self-Pay Journey Program; the page does not state the regular price for those two doses without an on-time refill. Lilly says the 2.5 mg and 5.5 mg prices “are temporarily reduced through December 31, 2026”, and defines a month as 30 days.

At the maintenance dose, then, the two tablets cost the same: $299 a month, with Foundayo’s figure conditional on refilling within 45 days. These prices cover medication only; you still need a prescriber, whose visit is billed separately unless a program bundles it. What GLP-1s cost in 2026 covers insurance, Medicare and the compounded alternatives, and what a GLP-1 program really costs per month adds program fees.

Who can get them?

Both labels indicate the tablet for chronic weight management in adults with obesity, or overweight with at least one weight-related condition, alongside a reduced-calorie diet and increased physical activity; do I qualify for a GLP-1? sets out what those criteria mean in practice and what programs ask. Our quiz shows whether you may meet common criteria; a result is not an approval, and a licensed clinician makes every prescribing decision.

Rybelsus is also an oral semaglutide tablet, but it is approved for type 2 diabetes, not weight loss, and its label says it is “not substitutable on a mg-to-mg basis” with Ozempic tablets. Compounded oral semaglutide sold by telehealth programs is a different product again: it is not FDA-approved, and the FDA says it does not review compounded drugs for safety, effectiveness or quality.

How to choose

Use decision rules, then let a clinician confirm.

The DoIQualify quiz asks whether you prefer a non-injection option and shows programs that offer one. The compare table marks which programs offer oral, injectable, compounded and brand-name medication.

Foundayo vs Wegovy pill FAQ

Is Foundayo better than the Wegovy pill?
Neither label says so: the figures come from each drug’s own placebo-controlled trial, not from a comparison between the two. In its own trial Wegovy 25 mg tablets averaged 13.6% weight loss at 64 weeks against 2.4% on placebo; Foundayo averaged 7.4%, 8.3% and 11.1% at 5.5, 9 and 17.2 mg over 72 weeks against 2.1%. Different patients, different lengths, so the figures are not a ranking. A licensed clinician makes every prescribing decision.
Can you take Foundayo with food?
Yes. Foundayo’s label says it can be taken with or without food, swallowed whole, and not broken, crushed or chewed. Wegovy tablets are the opposite: on an empty stomach in the morning with up to 4 ounces of water, then at least 30 minutes before eating, drinking or taking other oral medications.
How much do Foundayo and the Wegovy pill cost without insurance?
Both start at $149 a month at the manufacturers’ self-pay prices read on October 1, 2026. Novo Nordisk prices the Wegovy pill at $149 for 1.5 mg, $199 for 4 mg and $299 for 9 mg and 25 mg. Lilly prices Foundayo at $149 for 0.8 mg and 2.5 mg, $199 for 5.5 mg, $299 for 9 mg, and $299 for 14.5 mg and 17.2 mg when you refill within 45 days; it says the 2.5 mg and 5.5 mg prices are temporarily reduced through December 31, 2026.
Does Medicare cover Foundayo or the Wegovy pill?
For some members, through the Medicare GLP-1 Bridge, a temporary program running from July 1, 2026 to December 31, 2027 with a $50 monthly copayment. Medicare.gov lists Foundayo tablets and Wegovy tablets among the covered products. You need Part D, a prior authorization and one of the program’s BMI tiers.
