The BMI thresholds
Most GLP-1 programs follow the FDA labeling for weight-management medications. That means two common paths:
- BMI 30 or higher, with no other condition required.
- BMI 27 or higher plus at least one weight-related condition.
BMI is a screening number, not a diagnosis. It is the first thing a program checks because it is the first thing the label mentions. Calculate yours here.
Conditions that count at BMI 27+
Programs generally accept: pre-diabetes or type 2 diabetes, high blood pressure, high cholesterol, obstructive sleep apnea, and PCOS. Some also consider fatty liver disease and osteoarthritis. You'll usually be asked whether a doctor has told you about the condition, not for proof at intake.
What can disqualify you
Clinicians typically will not prescribe if you are pregnant, breastfeeding, or planning pregnancy; have a personal or family history of medullary thyroid cancer or MEN2; have a history of pancreatitis; or have an active eating disorder. Type 1 diabetes and certain GI conditions also lead to a decline in many programs.
The actual process
- Online intake: height, weight, history, current medications (10โ15 minutes).
- Clinician review, sometimes with a video or async message exchange. Some states require a synchronous visit.
- Lab work, if the program requires it (many don't at the start).
- Prescription sent to the program's pharmacy; first shipment typically arrives within a week.
